A: The most important record one could keep is a journal of symptoms, activity, appointments, and test results. It is also helpful to have your prior job description, and copies of any personal reviews – particularly if they are adverse.
It also helps to have objective evidence of impairments. For example, an exercise test that confirms poor exercise tolerance or neurocognitive testing that confirms cognitive dysfunction.
Supportive data accepted by Social Security include elevated EBV titers, a positive tilt test, an abnormal MRI, an abnormal sleep study, EEG, or SPECT scan. Acceptable documentation includes ongoing medical records and narrative information from relatives, friends, therapists, and counselors
